頭にくる[あたまにくる, atama ni kuru] be angry; become angry; get seriously pissed
Despite all the mild manners, good naturedness, polite demeanor, to be sure, the Japanese get angry just like the rest of us.
And when they do, whatever it was that got to them, got to their head.

If you see someone steaming, jumping up and down, ticked off, angry as all get out, fuming, boiling mad - take your pick, you can be sure that they used this phrase to say so.
The degree of angriness is expressed by the emotion in the voice.
